29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement.  a) 9839 Graphite Mountain Road, Hague, NY, on November 15, 2017, and at times prior, the footings were not sound, or rigid because the employer used varying pieces of small wood underneath the scaffold footing making the scaffolding unstable.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations and/or related work from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity):   a) 9839 Graphite Mountain Road, Hague, NY, on November 15, 2017, and at times prior, no guardrails or personal fall arrest system was used on portions of the scaffold working platform 30 feet above the ground.
