29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:   On the roof of this apartment complex building, North side, on or about December 1, 2017, and at times prior thereto, employees were exposed to struck-by injuries when handling metal decking and cleaning up roof debris were not required to wear safety glasses or gloves while cleaning up and handling materials.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On the roof of this apartment complex building, North side, on or about December 1, 2017, employees without fall protection were exposed to fall hazards greater than 6 feet to the concrete ground below, while installing roofing material on the roof.
