29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a) On or about December 4, 2017 and times prior at 5005 Montgomery Hwy, Dothan, Al: The employer failed to ensure the East stairway was maintained free of debris which exposed employees to slip/trip/fall hazards.    b) On or about December 4, 2017 and times prior at 5005 Montgomery Hwy, Dothan, Al: The employer failed to ensure bolts sticking up and capped 3 1/2 inches throughout the hallway on the 2nd floor were removed which exposed employees to slip/trip/fall hazards.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    a) On or about December 4, 2017 at 5005 Montgomery Hwy, Dothan, Al: The employer failed to ensure an employee was protected from falling when he used the stairway on the East end of the building due to the first level stairs were rebuilt over the weekend and no guardrails were installed to prevent falls.
