29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Powered industrial truck(s) which were used on a round-the-clock basis were not examined for defects after each shift:  a. On or before December 11, 2017, at the above addressed jobsite, powered industrial vehicles used daily were not inspected daily and/or after each shift.

29 CFR 1910.179(m)(1): A thorough inspection of all ropes were not being made at least once a month: Severe kinking, crushing, cutting, or unstranding.    a. On or about December 11, 2017, at the above addressed jobsite, the North 15 ton gantry crane located in the mini-casting department was not inspected monthly. There were several kinks in the running line, thereby exposing employees to a struck by hazard.

On or about December 11, 2017, in the facility, the emergency stop on the mini-casting drawing unit was not functioning, thereby exposing employees to hazards associated burns from molten metal blowout.
