29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a. Finishing Area - On or about May 28, 2014, employees servicing the Schumag 562 were exposed to an energized 480V panel with a broken disconnect switch. The condition of the switch was not in accordance with instructions included in the listing or labeling.

29 CFR 1910.303(f)(4): Capable of accepting a lock. Disconnecting means required by this subpart shall be capable of being locked in the open position. a. Finishing Area - On or about May 28, 2014 the energy isolation device for the Schumag 556 machine was not capable of being locked in the open position. Employees were thereby exposed to hazards associated with the unexpected energization of equipment.
