29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Shell car wash facility: An employee pulling Cat 5 cable on the roof was not protected by guardrail systems, safety net systems, or personal fall arrest systems. Employee was exposed to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  East side of Shell car wash facility:  An extension ladder used to access the roof did not extend at least three feet above the roof. Employee was exposed to fall hazards.
