29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about December 14, 2017, and at times prior, employees were exposed to caught-in hazards that could result in broken bones, during the task of clearing molds that get stuck in the descrambler and orienter that feed the molds into the blow mold machine. The box in which the descrambler and orienter are located, has open sides into which the operator can stick his or her hands while reaching for the interlocked door on the top of the box. This condition exists on both machine A and machine B. (b) On or about December 14, 2017, and at times prior, employees were exposed to caught-in hazards that could result in lacerations, falls to the ground, and other injuries, from the slack in the belt of the conveyors in the blow mold room, intermediate room, and Fuji room. The slack belt sags below the level of the guards on the conveyors. The panels in the belt can snag the smocks (lose clothing) of the employees who work adjacent to locations of the slack. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.303(b)(8)(i): Electrical equipment was not firmly secured to the surface on which it was mounted: (a) On or about December 14, 2017, and at times prior, employees were exposed to electrical hazards from an electrical junction box that was mounted with a zip tie to the frame of the conveyor in the intermediate room, rather than being mounted onto the frame with screws or other appropriate fasteners.
