The employer did not ensure that employees performing work below were protected with protective helmets, thus exposing employees to a head injury, on or about 12/18/17.

The employer did not ensure that appropriate means for access and egress of the scaffold was utilized, thus exposing employees to a fall hazard, on or about 12/18/17.
