29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.  On or about December 12, 2017:  a) At the facility located at 2331 Mellon Court in Decatur Georgia, the employer failed to ensure that an employee operating an electric style pallet jack was properly trained in accordance with the standard prior to be allowing to operate the pallet jack.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  On or about December 12, 2017:  a) At the facility located at 2331 Mellon Court in Decatur Georgia, the employer failed to ensure that powered industrial trucks including, but not limited to pallet jacks were removed from service until repairs were made when they were identified to have maintenance issues.  The Plaid Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(p)(1), which was contained in OSHA inspection number969332, citation number1, item number 2 and was affirmed as a final order on 10/06/2014, with respect to a workplace located at 2331 Mellon Court, Decatur, GA 30035.
