At the Mosaic site located at 4873 Port Sutton Rd., Tampa, FL, employees performing maintenance work were exposed to an ammonia release incident during the process of removing an 8" Orbit valve (MOV P2A) in the anhydrous ammonia system, on or about 12/14/2017. No means were used by the employer to assure that training provided to the employees was understood for the work to be conducted at the site.

The employees were issued and required to use 3M, Model 6800, full-face respirators, however, a medical evaluation was not provided to determine the employees' ability to use the respirators.

At the Mosaic site located at 4873 Port Sutton Rd., Tampa, FL, employees performing maintenance work to an anhydrous ammonia system valve were exposed to ammonia from an incidental release, on or about 12/14/2017. The company's Hazard Communication policy had not been implemented, in that, employees were not informed and/or trained on the health and/or physical hazards associated with chemicals such as, but not limited to anhydrous ammonia.
