29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  (a) Employee operating powered industrial vehicles (PIV) in the workplace had not been re-evaluated in the three years since their last documented training. One worker who operated the forklift on or about January 3, 2018 was last trained/evaluated on January 8, 2014, approximately 1 year past due.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) Shop area - Piranha Ironworker was not provided with a guard at the punch station of the machine. During the period between December 15, 2017 and May 24, 2018 shop employees processed material utilizing the punch station on the Piranha Ironworker exposing them to the potential of finger amputations.  (b) Shop area - Power Team 25-ton hydraulic punch was not provided with a guard to protect employees from amputation hazards by inadvertently placing their hand and or fingers in the danger area while punching holes in window and door frames.    (c) Shop area - DeWalt 17-inch radial arm saw used to cut miters in metal door and window frames was inadequately guarded in that the self adjusting blade guard was missing from the outer side of the saw blade. During the period between December 15, 2017 and May 24, 2018 shop employees processed material utilizing the DeWalt radial arm saw.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:  (a) Forklifts used in the facility were not inspected prior to being placed in service each day the lifts were used. On or about January 3, 2018, the Toyota lift truck was operated by an employee without a daily and/or pre-use inspection.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
