29 CFR 1910.184(f)(1): Wire-rope slings did not  have permanently affixed and legible identification markings as prescribed by the manufacturer  that indicated the recommended safe working load for the type(s) of hitch(es) used, the angle upon which it is based, and the number of legs if more than one:   Warehouse: The wire rope slings used for unloading bundles of rebar were missing permanently affixed legible identification markings stating the size, rated capacity for the type of hitch used and the angle upon it was based.   The Alvarez Industries, Inc. was previously cited for a similar violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.251(c)(16)which was contained in OSHA inspection number 1183282 citation number 01, item number 001a and was affirmed as a final order on 05/08/17, with respect to a workplace located at 299 Greenwich Avenue, Greenwich CT 06830.

29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.  Location:  312 Boston Post Road Orange, CT 06477  The employer did not record the following injury or illness incidents  on the 2017 OSHA 300 logs with seven (7) days of receiving information that it was recorded.  1. An injury resulting in six (6) days away from work on July 20, 2017 was not recorded on the OSHA log within seven (7) calendar days. 2. An injury resulting in forty-six (46) days away from work on November 9, 2017 was not recorded on the OSHA log within seven (7) calendar days.
