29 CFR 1910.37(a)(3): Exit route was not kept free and unobstructed:    a) On January 9th, 2018 and at times prior to, at the Dualco facility, the employer did not ensure an exit door around the west side area inside the shop was maintained in a free and unobstructed condition. The exit door was blocked with thirty paint buckets stacked on top of each other. This condition exposes employees to fire hazards.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:      (a) On January 9, 2018 and at times prior to, an employee working in the shop at the Dualco facility was exposed to struck by hazards while operating a Warner & Swasey 5 spindle automatic bar machine in that the employee did not have eye protection.    (b) On January 9, 2018 and at times prior to, an employee working in the shop at the Dualco facility was exposed to struck-by hazards while operating a Warner & Swasey Turret lathe no. 3 machine in that the employee did not have an eye protection.
