29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a. On or about January 12, 2018 two employees were installing laminated plywood roof sheathing on the exposed trusses of the structure using pneumatic nail guns and hand held power saws and were not wearing any eye protection exposing employees to eye and/or face hazards from flying wood particles or ricochet nails.    b. On or about January 12, 2018 one employee was using a Makita Brand electric 7-1/4 inch circular saw to cut 2x4 studs to size and was not wearing any eye protection while operating the saw.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a. On or abut January 12, 2018, employees were using a 12/3 wire flexible extension cord at the front door, powered by an ungrounded portable generator in the street, to energize electric hand circular saws and were exposed to electrical shock hazards because the ground pin of the extension cord had been removed or broken off eliminating the continuous path to ground.

On or about January 12, 2018 four employees were installing truss blocking and roof sheathing on three new one story residential structures at lots 86, 88 and 89 and were not protected by any means of personal fall protection, guardrails or safety nets exposing them to falls in excess of 8 feet to the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      a. On or about January 12, 2018 four employees were working on the roof tops of three structures at 2798 Gibraltar Drive, New Smyrna Beach, FL using three portable ladders to gain access to the roof. Two of the aluminum extension ladders did not extend above the roof surface three feet to assist employees in mounting and dismounting the ladder.    b. On or about January 12, 2018, one employee utilized a 6 foot portable extension ladder to gain access to the roof trusses. This ladder was not long enough to extend three feet above the roof line, to assist employees in mounting and dismounting the ladder.

On or about January 12, 2018 one employee had utilized the top step of a 6 foot A frame portable ladder to gain access to the top of the CMU wall 8 feet above the concrete floor, and climb up onto the roof truss system.
