29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.  Location: 3 Rose Court, Bayonne, NJ.  a)  Employees using an extension ladder at the worksite did not ensure a proper ladder angle. The base distance of the ladder was not 1/4 of the working ladder's length.  The site layout and/or physical conditions  where the extension ladder was erected (concrete walkway between 3 Rose Court and 4 Rose Court) were approximately 6.42 ft. base distance and the working ladder's length was 28.72 ft.; on or about 1/18/18.
