29 CFR 1910.28(b)(6)(i):  Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system:  At 8925 Zev Avenue, Berkeley, MO, specifically inside of the Heat Treatment Area, employees were not adequately protected from falling into two pits.  The first pit, containing a molten salt bath, measured approximately 77 inches long by 53 inches wide with a 34 inch high coaming along all four sides.  The second pit, containing water, measured approximately 92 inches long by 56 inches wide with a 22 inch high coaming along all four sides.  The depth of each pit depth was approximately five feet.  A travel restraint system was not provided to protect these employees from falling in to either pit.  This violation was observed on 01/31/18.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  At 8925 Zev Avenue, Berkeley, MO, specifically in the Fabrication Department, the Apex Circular Sander was not adequately guarded.  The unguarded portions of this circular sander measured approximately 20 inches in diameter.  This violation was observed on 01/31/18.  At 8925 Zev Avenue, Berkeley, MO, specifically inside of the Shear Department, the ten stock holders belonging to the Cincinnati Shear, m/n 10-10, s/n 20715, were inadequately guarded.  Each stock holder was in front of the finger guard and approximately 3/4" above the shear table.  This violation was observed on 01/31/18.  At 8925 Zev Avenue, Berkeley, MO, specifically inside of the Forming Department, the rotating parts belonging to a Bosdick Radial Arm Drill were unguarded.  This violation was observed on 01/31/18.
