29 CFR 1904.31(a): Recordable injuries and illnesses of all company employees including labor, executive, hourly, salary, part-time, seasonal, or migrant workers and employees who were not on the company payroll but supervised by the employer were not recorded on the OSHA 300 log.    On or about January 25, 2018, the employer failed to establish and maintain the OSHA 300/300A logs.  There was no record of any logs being established or kept.  The most recent injury, per the employer, was a laceration which required sutures that occurred approximately three weeks prior to this inspection.
