29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative: (a)Preform Solutions, Inc. at 1015 W. Blackhawk St., Sioux Falls, SD 57104: On or about and at times prior to January 26, 2018, employees were exposed to injury in that the employer had not implemented an effective energy control program for the following injection molding equipment: (1)Krauss Maffei (2)Sumitomo (3)Husky Powerline 330 (4)Nissei Abatement Note: As part of an energy-control program, employers must: 1)Establish energy-control procedures for removing the energy supply from machines and for putting appropriate lockout or tag-out devices on the energy-isolating devices to prevent unexpected re-energization. When appropriate, the procedure also must address stored or potentially re-accumulated energy. 2)Train employees on the energy-control program, including the safe application, use, and removal of energy controls; and 3)Inspect these procedures periodically (at least annually) to ensure that they are being followed and that they remain effective in preventing employee exposure to hazardous energy.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as servicing and/ or maintenance of equipment: (a)Preform Solutions, Inc. at 1015 W. Blackhawk St., Sioux Falls, SD 57104: On or about and at times prior to January 26, 2018, employees performing maintenance in the injection molding area were exposed to caught-in and struck-by hazards in that specific instructions related to de-energizing equipment were not developed. This injection molding equipment includes the following: (1)Krauss Maffei (2)Sumitomo (3)Husky Powerline 330 (4)Nissei Abatement Note: The procedures shall include but are not limited to the following: (1)A specific statement of the intended use of the procedures; (2)Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (3)Specific procedural steps for the placement, removal and transfer of lockout devices or tag-out devices and the responsibility for them; and (4)Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tag-out devices, and other energy control measures.
