29 CFR 1910.28(b)(3)(i): (3) Holes.  The employer must ensure: (i) Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following:  (1) Covers; (2) Guardrail systems; (3) Travel restraint systems; or  (4) Personal fall arrest systems  a. On or about January 30, 2018, located on the ladle relining platform, there was a hole that was approximately 14 feet in diameter that had a single chain in place that did not protect the employees from a fall, thereby exposing the employees to a fall hazard of approximately 9 feet 6 inches.
