29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings:    (a)  On or about January 29, 2018, and at times prior, workers were exposed to trip and fall hazards on the fixed stairs on the outside of the office building next to the truck scale in that the fifth step of seven, was 9 inches deep while all of the other steps were 12 inches deep. 29 CFR 1910.29(b)(1): A top rail, or equivalent guardrail system member, 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface was not installed:    (a)  On or about January 29, 2018, and at times prior, the top rail was missing on the platforms affixed to the office building, from which workers take samples of product from trucks that enter the facility.  The platforms were approximately 6 feet and 8 feet above the ground. 29 CFR 1910.305(j)(2)(v): A receptacle installed outdoors in a location protected from the weather or in other damp locations did not have an enclosure for the receptacle that was weatherproof when the receptacle was covered (attachment plug cap not inserted and receptacle covers closed):  (a)  On or about January 29, 2018, employees were exposed to electrical hazards from an outlet box which was missing the receptacle cover.  The box is located outside next to the ground storage elevator, and is used to plug in diesel motors during cold weather conditions. 29 CFR 1910.1200(f)(6):  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with either the information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:    (a)  On or about January 29, 2017, and at times prior, the employer did not ensure that containers of hazardous chemicals had labels to inform employees about hazards of the chemicals in the containers. 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  (a)  On or about January 29, 2018, and at times prior, the employer did not ensure that employees were safe from hazardous energy by conducting annual inspection of each energy control procedure in use at the facility.    Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").  Abatement Note:  The periodic inspection shall be performed by an authorized employee other than the ones(s) utilizing the energy control procedure, for the purpose of correcting any deviations or inadequacies identified.  The periodic inspection shall include a review of each authorized and affected employees' responsibilities under the procedure.  The employer shall certify the periodic inspections by identifying the machine or equipment on which the procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.
