On or about February 01, 2018, at the jobsite located 5409 Bulwer Avenue, Saint Louis, Missouri, the employer did not ensure the exit route to the courtyard in Building 3 was free and unobstructed. The emergency exit route was blocked by the storage of materials handling carts; tires; cardboard boxes; folding chairs, and other discarded equipment. Exit access was completely blocked.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or did not have smooth, well-rounded surfaces on which the cords may bear. On or about February 01, 2018, at the jobsite located 5409 Bulwer Avenue, Saint Louis, Missouri, the employer did not ensure each outlet had a cover, faceplate or fixture canopy. In the downstairs breakroom room, the live electrical outlet below the sink was missing a faceplate.
