29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about February 2, 2018, employees performing siding installation on a single-family property were exposed to struck-by hazards when the employer failed to mandate the use of hard hats while employees worked at two different levels.  No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about February 2, 2018, employees performing siding installation on a single-family property were exposed to a 12.8 foot fall when the employer failed to ensure that fall protection was utilized.   No abatement certification or documentation is required of this item.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about February 2, 2018, employees conducting siding installation on a residential single family home were exposed to struck-by hazards when the employer failed to mandate the use of eye protection while operating a pneumatic nail fastener.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1181932, citation number 1, item number 1 and was affirmed as a final order on December 15, 2016, with respect to a workplace located at 4444 Wilson Ave., Downers Grove, Illinois 60515.  No abatement certification or documentation is required for this item.
