Faces Leaps & Bounds Building (Wing 600) - On or about and times prior to February 15, 2018, employees forearms were not protected during welding operations, thus exposing employees to a thermal hazard.

When a cable, other than the cable lead referred to in paragraph (b)(2) of this section, becomes worn to the extent of exposing bare conductors, the portion thus exposed was not protected by means of rubber and friction tape or other equivalent insulation:  a). Faces Leaps & Bounds Building (600 Wing) - On or about February 15, 2018, the welding cable on a Lincoln Electric 250 GXT Ranger welder machine had bare wiring exposed, thus exposing an employee to an electrical hazard.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a). Faces Leaps & Bounds Building (600 Wing) - On or about and times prior to February 15, 2018, employees were welding metal roof panels onto the building's roof without a fall protection system in place, thus exposing employees to a fall hazard of approximately 26 feet.
