29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about February 28, 2018, on the west side of the commercial construction where an employee was exposed to a fall hazard greater than 6 feet when performing work on the roof without a fall protection system.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.  On or about February 28, 2018, on the west side of the commercial construction, where an employee was exposed to a fall hazard when accessing the roof using the top step of a stepladder as a step for which it was not designed.
