29 CFR 1910.141(a)(5): The employer did not ensure the enclosed workplace was constructed and maintained to prevent the entrance or harborage of rodents. A continuing and effective extermination program was not implemented after the detection of rodents in the workplace: On or about March 14, 2018, employees were exposed to health hazards from mice and other rodents and the workplace was not maintained to prevent entrance of mice through a hole in the wall and the employer did not implement an effective extermination program.

29 CFR 1910.303(b)(2): The employer did not ensure listed or labeled electrical equipment was used or installed in accordance with instructions included in the listing or labeling. On or about March 14, 2018, employees were exposed to fire and electrical hazards from a relocatable electrical power tap used for powering multiple electronic devices. The relocatable power tap was not connected to a permanently installed branch circuit receptacle outlet and was connected in series to another extension cord.
