29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: (a) On or about March 13, 2018, an employee was exposed to shock, burn, and other electrical hazards while using an extension cord to power a circular saw, which had a cut in the insulation exposing the live conductors inside. 29 CFR 1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear: (a) On or about March 13, 2018, Lakota Roofing & Construction Inc., as controlling employer, did not ensure that employees of Alvaro Coto Delgado dba Delgado Services, subcontractor, were protected from falls of greater than 19 feet while accessing a ladder at the roof's edge in that the top of an adjacent ladder created a trip hazard at the landing surface. 29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on a low-slope roof with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) On or about March 13, 2018, Lakota Roofing & Construction Inc., as controlling employer, did not ensure that employees of Alvaro Cota Delgado dba Delgado Services, subcontractor, were protected from falls of greater than 19 feet while reroofing a flat roof and working near the edge. An improperly erected warning line system was the only method of fall protection used on the roof. The warning line system was erected less than 6 feet from the edges of the building and had no point of access. Lakota Roofing & Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1017958, citation number 1, item number 1, and was affirmed as a final order on March 15, 2015, with respect to a workplace located at 2900 S. Peoria Street in Aurora, Colorado. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
