On or about March 15, 2018, located at 1528 North State Street in Westerville, Ohio , employees were constructing a residential home where there was a floor hole for a recessed bathtub near that was approximately 4 - 1/2 feet by 7 feet in size, thereby exposing the employees to a fall hazard of approximately 10 feet.  d. On or about March 15, 2018, located at 1528 North State Street in Westerville, Ohio , employees were constructing a residential home where there was a floor hole in the garage for future stair access to the basement, thereby exposing employees to a fall hazard of approximately 10 feet.   e.
