The door connecting to the single exit route swung into the room that was used for storage of combustible and flammable materials.

Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  a)  For employees working at 2009 North Florida Avenue in Tampa, in the main front storage room, on or about March 22, 2018. The material was stacked in unstable piles while the employees were working exposing the employees to a struck by hazard.

At the main power panel that housed the breakers for the electrical components installed in the building, two unused circuit breakers were missing covers, exposing the energized main bus bar.  b) For employees working at 2009 North Florida Avenue in Tampa, on or about March 22, 2018. In the rear area storage an outlet box and a junction box did not have the unused knockout openings closed, exposing employees to electrical hazards.

29 CFR 1910.305(a)(2)(ix): Lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard:  a)  For employees working at 2009 North Florida Avenue in Tampa, in the rear area storage room, on or about March 22, 2018. The lamps in the rear storage area did not have covers on them to protect them from contact or breakage. The lights were the only illumination for this room and had to be turned on and off by pull strings.
