29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a. Metro Design, Inc. - Elyria, Ohio:  On or about March 27, 2018, the employer did not ensure that employees who setup and maintain the Prodigy CNC lathes such as, but not limited to, machine numbers 6, 10, and 27, were trained in the skills required for the safe application, usage, and removal of energy controls.  Employees who changed tooling and set indicators were not trained to isolate and/or render energy sources safe.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a. Metro Design, Inc. - Elyria, Ohio: On or about March 27, 2018, employees were exposed to rotating parts and pinch point hazards in that the #6 Prodigy CNC lathe (s/n 20-4128-S) was not deenergized prior to set-up and servicing tasks such as, but not limited to, tooling changes.  The employer did not ensure that energy control application steps were implemented to control hazardous electrical/mechanical and pneumatic energy prior to the servicing tasks  the employer did not:   -  Ensure employees had the method or means to control hazardous energy.  -  Ensure that equipment was turned off or shut down.  -  Physically operate energy isolating devices to control the energy source(s).   -  Verify that isolation and de-energization of the machine or equipment had been accomplished.    b. Metro Design, Inc. - Elyria, Ohio: On or about March 27, 2018, employees were exposed to rotating parts and pinch point hazards in that the #27 Prodigy CNC lathe (s/n s/n 20-4320-S) was not deenergized prior to set-up and servicing tasks such as, but not limited to, tooling changes.  The employer did not ensure that energy control application steps were implemented to control hazardous electrical/mechanical and pneumatic energy prior to the servicing tasks  the employer did not:   -  Ensure employees had the method or means to control hazardous energy.  -  Ensure that equipment was turned off or shut down.  -  Physically operate energy isolating devices to control the energy source(s).   -  Verify that isolation and de-energization of the machine or equipment had been accomplished.    c. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. Metro Design, Inc. - Elyria, Ohio: On or about March 27, 2018, employees operating the Topper vertical CNC lathe (s/n 011251) were exposed to injuries from the machine's rotating parts, point of operation, and/or flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard area during operation.       b. Metro Design, Inc. - Elyria, Ohio: On or about March 27, 2018, employees operating the Upgrade Technologies machine #2 located in the back room were exposed to injury from the machine's rotating parts, point of operation, and/or flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard areas during operation.       c. Metro Design, Inc. - Elyria, Ohio: On or about March 27, 2018, employees operating the #9 Prodigy CNC lathe (s/n 20-4186-S) located in the back room were exposed to injuries from the machine's rotating parts, point of operation, and/or flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard areas during operation.       d. Metro Design, Inc. - Elyria, Ohio: On or about March 27, 2018, employees operating the #6 Prodigy CNC lathe (s/n 20-4128-S)  located in the front room were exposed to injuries from the machine's rotating parts, point of operation, and/or flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard areas during operation.       e.
