29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) North side of building #3 - On or about March 28, 2018 an employee operating a JLG aerial lift was not protected from being ejected out with the use of a body belt and lanyard attached to the lift basket.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) West side of house on third floor - On or about March 28, 2018 an employee was walking on top of the framed roof rafters installing blocking 40 feet high without any fall protection.
