29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:   a)  At 11545 Forsyth Loop, Spanish Fort, Alabama:  On or about March 5, 2018 and at times prior; the employer allowed his employee to operate a skill saw on site with a wood wedge holding the guard up exposing the blade and exposing his employee to amputation hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 11545 Forsyth Loop, Spanish Fort, Alabama:  On or about March 5, 2018 and at times prior; the employer allowed his employee to perform roof decking activities without fall protection exposing him to a fall hazard of up to 20 feet 10 inches to the concrete floor below.
