29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :    a) Main shop floor, 69 Norman Street, Everett    On or about April 10, 2018, an employee working on the main floor of the facility was exposed to continuous noise levels at 136.4% of the permissible daily noise exposure (8-hour time-weighted average sound level of 80 dBA) during the 459 minute sampling period on April 19, 2019; exposure calculations included a zero increment for the 21 minutes not sampled. This employee was not provided with an annual audiometric examination.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    a) Main shop floor, 69 Norman Street, Everett    On or about April 10, 2018, the employer did not ensure that a nitrogen tank located adjacent to the pickling tanks was capped, in accordance with section 3.4.3 of the Compressed Gas Association Pamphlet P-1 1965. The tank was free standing on the shop floor, with the regulator attached.     There is no abatement certification or documentation required for this item.
