29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  The employer is failing to ensure that employee's eyes and face are protected from flying particles while working on a residential roof.  This most recently occurred on April 11, 2017 on the jobsite located at 3315 N 147th St, Omaha, NE 68116.  Three employees were observed using hand-held pneumatic nail guns to install roofing material without any form of eye or face protection.  Employees were working on two single-story garage units at the Belle Meade Condominium complex.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on April 11, 2017 on the jobsite located at 3315 N 147th St, Omaha, NE 68116.  Ten employees were re-roofing two single-story garage units and not utilizing any form of fall protection.  Garage units have a five in twelve steep sloped pitch and ground to eve height is approximately eight feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer is failing to ensure that each employee exposed to fall hazards has been effectively trained on fall hazard recognition and the use fall protection.  This most recently occurred on April 11, 2017 on the jobsite located at 3315 N 147th St, Omaha, NE 68116.  Ten untrained employees were working on a residential roof without fall protection or an understanding of the requirements for working at heights greater than six feet.
