Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did report the in-patient hospitalization, amputation, or loss of an eye to OSHA:  a) Facility, 200 Braddock Avenue, Turtle Creek, PA:  On April 4, 2018 at 1:30 PM, an incident occurred and an employee was taken to the hospital.  The employer did not report it to OSHA until April 6, 2018 at 1:10 PM.  Two weeks later, the employee is still in the hospital.
