29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    a) Jobsite, 200 Braddock Avenue, Turtle Creek, PA:  On or about April 4, 2018, an employee was attempting to move several 6' in diameter, 3/4" plywood pieces from a railing that they were leaning against to transport them, when after moving the first one, the others tipped back, and approximately 9 of them, weighing 130 pounds each, pinned the employee against the door of the forklift.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did report the in-patient hospitalization, amputation, or loss of an eye to OSHA:  a) Facility, 200 Braddock Avenue, Turtle Creek, PA:  On April 4, 2018 at 1:30 PM, an incident occurred and an employee was taken to the hospital.  The employer did not report it to OSHA until April 6, 2018 at 1:10 PM.  Two weeks later, the employee is still in the hospital.
