On or about April 17, 2018, the supervisor and an employee used a 6-foot portable "A" frame ladder that did not extend at least 3 feet above the upper landing surface to gain access to the top plate of the first level of the 2-story multi-unit residence, exposed to 6-foot and 8-foot fall hazards.
