29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   A.  On or about May 1, 2018, Machine Shop:  The point of operation at the Haas DS 30SS CNC Lathe was not effectively guarded, in that the interlock had been defeated.  The condition allowed the enclosure door to open during lathe operation, exposing employees to laceration, amputation and entanglement hazards.
