29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):    a) Jobsite, 208 Haines Road, Latrobe, PA:  On or about April 20, 2018, employees had erected a scaffold that was approximately 3-4 feet away from what is believed to be 220 volt energized power lines.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    a) Jobsite, 208 Haines Road, Latrobe, PA:  On or about April 20, 2018, 4 employees were working on a residential roof at heights of 24-35 feet without fall protection and the owner did not have a signed certificate documenting they had been trained in fall protection.
