29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:       (a) On or about April 24, 2018 and at times prior to that date, for the employee exposed to amputation hazards while operating the glass carousel, company asset number 130400 where the roller arm and machine frame created a shear point in the component department located at 1580 South 48th Street, Grand Forks, North Dakota.       (b) On or about April 24, 2018 and at times prior to that date, for the employee exposed to amputation hazards while operating the glass carousel, company asset number 090135 where the roller arm and machine frame created a shear point in the component department located at 1580 South 48th Street, Grand Forks, North Dakota.
