29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) 425 Network Station Rd., Ste 50, Chesapeake, VA 23320: On or about April 26, 2018, and times prior thereto, a work rest on the Dayton 8" abrasive wheel grinding machine used to grind fasteners was up to approximately 1/4" from the wheel, exposing employees to struck-by and caught-in hazards.  No abatement certification or documentation required.

29 CFR 1915.89(b): A written program and procedures for lockout and tags-plus systems to control hazardous energy that included the provisions in 29 CFR 1915.89(b)(1) - (b)(6) was not established and implemented while servicing machinery, equipment or systems:  a) 750 W. Berkley Ave., Norfolk, VA 23253, U.S.S. Vicksburg: On or about June 12, 2018, and times prior thereto, the employer did not have a written program for lockout/tags-plus while servicing air conditioning units with electrical and water energy sources, exposing employees to electrical and struck-by hazards. Navy ship's force maintained control of the machinery, equipment or systems on the vessel.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.32(b)(2)(ii): The employer did not complete on the annual summary, the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log:  a) 425 Network Station, Ste 50, Chesapeake, VA 23320: On or about April 26, 2018, and times prior thereto, the employer did not complete the 300A Summary for 2017 for the annual average number of employees and the total number of hours worked by all employees at the establishment.  Abatement documentation is required for this item.
