29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. A) Exterior, Front of house: Employees were exposed to fall hazards of approximately 14 feet while using an extension ladder that was not extended at least 3 feet over the landing in which they were accessing, on or about April 24th, 2018.
