29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)Big Sioux Construction, LLC at 4615 Techlink Circle, Sioux Falls, SD 57106: On or about and at times prior to April 25, 2018, the exposing employer did not ensure employees were protected from fall hazards while installing a parapet wall on the exterior edge of a flat roof on a commercial building under construction at heights greater than 6 feet above a lower level. This condition exposed an employee to 12-foot fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)Big Sioux Construction, LLC at 4615 Techlink Circle, Sioux Falls, SD 57106: On or about and at times prior to April 25, 2018, the exposing employer did not ensure employees were protected from fall hazards when accessing a flat roof greater than 6 feet above a lower level in that, the portable extension ladder used to access the flat roof was not extended at least 3 feet above the upper landing surface. This condition exposed an employees to a 12-foot fall hazard.
