All repairs shall be made by authorized personnel.  a) On April 20, 2018 the brakes on the fork lift were found to be defective and was not taken out of service until April 25, 2018.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  a) The employer did not ensure that daily inspections were being performed on the warehouse fork lift in that they were only being done every 2-3 months.
