29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for lack of guard rails by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    Housing Unit #208:  Competent person did not inspect the scaffolding every shift.  Missing guard rails were not addressed prior to employees being exposed fall hazards greater than 10 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   Employees were exposed to fall hazards and were not provided with training to recognize and minimize those hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about April 26, 2018, employees engaged in roofing activities were exposed to an 11 feet fall hazards on a 4:12 pitched roof without the use of a fall protection system.
