29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   a) On or about April 30, 2018, employees were exposed to fall hazards of approximately 25 feet while working on the mobile scaffold to bring roofing materials on the roof. The mobile scaffold was missing a guard rail system on one side.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:   a) On or about April 30, 2018, employees were exposed to fall hazards of approximately 25 feet while working on the roof of a two-story residential house. The employees were not protected with a fall protection system.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):   a) On or about April 30, 2018, employees were exposed to fall hazards of approximately 25 feet while using an extension ladder to access and egress the roof of a two-story residential house. The horizontal distance between the foot of the ladder and the roof was approximately 4 feet. The horizontal distance should be at least 6 feet (one-quarter of 25 feet).
