Location: a) On the east side of the fabrication shop on the top tier linen fabrication area: On or about 04/30/18, employees access the top tier linen fabrication area by walking on an approximately 0.5 inches thick plywood that span approximately 48 inches between two galvanized steel floor joist. The employer did not ensure that the walking-working surface can support the maximum intended load. Employees were exposed to fall hazard due to structure collapse.

The top tier shelving collapsed, and the wood planks on the tier fell onto the employees.
