On or about May 3, 2018, the employer, PNK (Ohio), LLC dba Belterra Park Gaming and Entertainment Center, did not record the following workplace injuries and illnesses on the OSHA 300 Log for the calendar year 2018.  a)  On April 7, 2018, an employee lost consciousness at work and the employer did not record the incident with a check mark in Column J.  Abatement Certification and Documentation is required
