29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   Beverage Bar: On or about March 10, 2026, employee(s) used chemicals such as, but not limited to, ProPower and ProClean cleaning products, without using proper eye protection.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  Employee Dining Room: On or about March 10, 2026, employees were using Innorvera 6-Outlet Relocatable Power Taps that were series connected one to another.  Apache Sky Casino was previously cited for being in violation of 29 CFR  1910.303(b)(2), which was contained in OSHA inspection number 1624632, citation number 1, item number 1-b and was affirmed as a final order on 12/12/2022, with respect to a workplace located at 777 Apache Sky Blvd, Winkelman, AZ 85192.
