29 CFR 1910.134(c)(1)(vi): The written program did not contain procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators: Basement area B1 On or about May 3, 2018, employees were required to wear supplied air hood-type respirators during spray foam coating operations, and the employer failed to ensure that the written program contained procedures to prevent the entry of carbon monoxide in to the Bullard portable free air pump (Model EDP16TE) inlet.

29 CFR 1910.134(e)(1) as required by 1926.103: The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: Basement area B1 On or about May 3, 2018, employees were required to use supplied air hood-type respirators during spray foam coating operations, and the employer failed to ensure that the respirator user was medically qualified to wear the hood-type respirator.
