29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage and sharp corners and projections were not avoided:        a) Job site, 8651 Carey Lane, Pittsburgh, PA: On or about May 15, 2018, during construction activities, the employer did not ensure that cords were used appropriately in that they were strewn on the floor in the walking/working areas where they were subjected to being stepped on, and they were exposed to sharp edges on metal stud framing and/or framing components.

29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:      a) Job site, 8651 Carey Lane, Pittsburgh, PA: On or about June 11, 2018, during construction activities, the employer did not ensure that guardrail systems at the site were appropriate to be used for fall protection. Deficiencies noted during the inspection included, but were not limited to, at least one wire rope guardrail section was found to be 38" in height and some wire rope guardrail sections were able to deflect beyond 3 inches in an upward or downward direction, exposing employees to fall hazards of up to approximately 30 feet.

Deficiencies noted during the inspection included, but were not limited to, electrical, housekeeping and fall protection hazards.
