29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) On or about May 10, 2018, the controlling and exposing employer, Kumar & Associates, Inc., did not ensure their employee and two employees of Joe's Drilling, LLC, sub-contractor, were protected from electrical hazards where their employee was in the vicinity of Joe's Drilling employees who were permitted to work directly under an energized 12,470 volt power line. Exposure to this hazard resulted in the electrocution of two employees of Joe's Drilling, LLC. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.416(a)(3): Employer did not advise employees of the location of energized electrical power circuits, the hazards involved, and the protective measures to be taken: (a) On or about May 10, 2018, the controlling and exposing employer, Kumar & Associates, Inc., did not advise their employee or the two employees of Joe's Drilling, LLC, sub-contractor, of the hazards involved with the energized 12,470 volt power line located directly above the in-ground drilling location. Exposure to this condition resulted the electrocution of the two employee's of Joe's Drilling, LLC. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
